慢性阻塞性肺疾病(COPD)是临床最常见的疾病之一,持续气流受限致肺通气功能障碍,通气与血流比例失调和弥散障碍共同作用是其主要特点。由于经鼻高流量氧疗(HFNC)可增加呼吸末正压,冲刷咽部死腔,减少二氧化碳(CO2)的重吸入,提高肺泡通气量,且具有舒适度高等优势,已成为部分COPD患者的理想选择,但对于哪些COPD患者获益最大,目前还缺乏更多的循证医学证据。本文回顾整理了近几年主动温湿化的HFNC氧疗在COPD患者中应用的文献资料,进行综述。

Chronic obstructive pulmonary disease (COPD) is one of the most common diseases, which is characterized by persistent airway obstruction, ventilation-perfusion imbalance and diffusion disorder. With the therapeutic advantages of increasing positive end-expiratory pressure, washing out the air in the pharyngeal dead space, reducing CO2 re-inhalation, improving alveolar ventilation, and better comfort, high-flow nasal cannula (HFNC) oxygen therapy has been applied as the first-line oxygen therapy strategy in COPD patients. However, there is still not enough evidence to support which kind of COPD patients will benefit from HFNC most and we reviewed recent studies in application of HFNC in COPD patients in this paper.
