目的

调查在中国大陆中南地区ICU医师和护士职业倦怠现状及其危险因素。

方法

基于网络电子问卷选取国内中南地区在ICU工作的医师和护士,问卷包括职业倦怠量表及一般人口学特征量表,对收集的数据进行描述性分析和回归分析。

结果

此次调查共发放调查问卷表1389份,返回合格问卷1369份,回收率98%。其中医师687名,护士682名。中国大陆中南地区ICU医师和护士职业倦怠的总体阳性率为86.19%,严重职业倦怠阳性率为41.86%;逻辑回归结果显示职业倦怠的独立危险因素包括收入不满意、担心自身安全、治疗选择有困难、不了解职业倦怠;而职称为中级、不热爱ICU工作、夜班频率<7 d、节假日值班无补偿、担心自身安全是严重职业倦怠的独立危险因素。

结论

ICU医师和护士职业倦怠阳性率较高,职业倦怠的发生与多因素有关;提高医师和护士人身安全保障可能会降低职业倦怠发生率,职业倦怠者离职意愿较强,建议对其加强心理疏导和普及职业倦怠相关知识。

Objective

To investigate the status and risk factors of medical burnout in intensive care unit (ICU) in south-central area of China mainland.

Methods

Doctors and nurses working in the ICU in south-central area of China were selected based on online questionnaires, including burnout inventory and general demographic characteristics inventory. Descriptive and regression analysis were conducted.

Results

A total of 1389 questionnaires were issued and 1369 qualified questionnaires were returned, with a recovery rate of 98%, including 687 doctors and 682 nurses. The overall positive rates of burnout and severe burnout in the south-central area of Chinese mainland were 86.19% and 41.86, respectively. Logistic regression results showed that independent risk factors for burnout including income dissatisfaction, worried about security, difficulty in making treatment choices and not know about burnout. In addition, the mid-level title, frequency of night shift in ICU less than 7 days, duty on holidays without compensation, worried about security were independent risk factors for severe burnout.

Conclusion

The positive rate of burnout was high in the intensive care unit, and the occurrence of burnout was related to multiple factors. Improving the personal security of medical staff may reduce the incidence of burnout, and the burnout patients have a strong turnover intention. It is suggested to strengthen psychological counseling and popularize relevant knowledge of burnout.
