目的

探讨ICU后综合征(PICS)研究热点及研究趋势,以期为我国PICS研究的发展提供参考。

方法

以“post-intensive care syndrome”为主题词,借助CiteSpace对2000年1月1日至2022年12月31日Web of Science核心集数据库检索的相关文献进行分析,梳理国家、研究机构和文献作者的发文情况、主要研究热点及趋势。

结果

共纳入2643篇PICS相关文献,发文量前3位的国家分别为美国、英国和加拿大;发文量前3位的研究机构分别是约翰霍普金斯大学、范德比尔特大学、多伦多大学;发文量前3位的作者分别是Dale M. Needham、Ramona O. Hopkins、E. Wesley Ely。关键词突变性分析主要为生活质量、严重脓毒症、危重症患者、急性肾衰竭、机械通气延长、幸存者。文献共被引及聚类分析,共形成13个聚类,主要集中于PICS的好发人群,如急性肾损伤、急性肺损伤、多器官功能衰竭、脓毒症休克,以及其临床症状,如ICU获得性衰弱、精神病学症状、虚弱等。根据高被引文献分析,研究主要集中在急性呼吸窘迫综合征(ARDS)、休克等危重症幸存者结局及生活质量、抑郁、创伤后应激障碍等症状的流行病学特点、PICS的概念及其预防管理策略。

结论

不同危重症幸存者PICS的流行病学特征、影响因素和远期结局,不同维度症状的长期轨迹变化特征、远期生活质量的评估及调查、预防管理策略为目前国际研究热点及方向。

Objective

To explore research hotspots and trends in post-ICU syndrome in the world and provide guidance and theoretical basis for the future research.

Methods

CiteSpace scientific knowledge map was used to analyze literatures retrieved from the Web of Science core collection database from 2000 to 2022 in post-ICU syndrome.

Results

A total of 2643 papers related to post-ICU syndrome were included, in terms of number of articles, the top 3 countries were USA, UK and Canada; the top 3 research institutions were Johns Hopkins University, Vanderbilt University, University of Toronto; the top 3 authors were Dale M. Needham, Ramona O. Hopkins, E. Wesley Ely. Keyword with strongest bursts including quality of life, severe sepsis, critical ill patient, acute renal failure, prolonged mechanical ventilation, survivors. Literature co-citation and cluster analysis resulted in 13 clusters that focused on the well-prevalent population of post-ICU syndrome, such as patients with acute kidney injury, acute lung injury, multi-organ failure, or septic shock, and on clinical symptom, such as ICU-acquired weakness, psychiatric symptoms, and frailty. According to analysis of top cited literature, research hotspots focused on epidemiology characteristics about outcome and quality of life in survivors of ARDS and shock patients, on depression, on post-traumatic stress disorder, and on concept of post-ICU syndrome and preventive strategies.

Conclusion

The epidemiology characteristics and long-term outcome of PICS in survivors of critical illnesses, the characteristics of long-term trajectory of PICS, assessment of long-term quality of life and preventive strategies of PICS are hotspots and future directions in international research.
