目的

探讨中国公民器官捐献供体的病原学特征,为控制供体感染制定正确的策略提供理论依据。

方法

采用回顾性对照研究方法,纳入2014年10月至2017年3月于中山大学附属第一医院公民器官捐献供体的临床资料148例,分析供体感染病原学特征。

结果

148例供体共分离出119株病原体。其中革兰阴性杆菌59株,占49.6%,检出率最高的分别是鲍曼不动杆菌、肺炎克雷伯菌、铜绿假单胞菌;革兰阳性球菌47株,占39.5%,检出率最高的分别是表皮葡萄球菌、金黄色葡萄球菌、头状葡萄球菌;真菌13株,占10.9%,分别是白色念珠菌、热带念珠菌、黑曲霉菌。检出部位分别在肺部、血液和泌尿系统。其中肺部感染主要病原菌为鲍曼不动杆菌,血行感染主要为表皮葡萄球菌,而泌尿系统则以热带念珠菌检出率最高。

结论

我国公民器官捐献供体感染的总体发生率很高,病原学涉及细菌和真菌。临床上对供体器官的维护需要进一步加强对其抗感染的力度。

Objective

To investigate the pathogenic characteristics of Chinese organ donors and provide the theoretical basis for infection control.

Methods

A retrospective analysis of data from October 2014 to March 2017 was performed. The pathogenic characteristics of 148 organ donors from the First Affiliated Hospital, Sun Yat-sen University were analyzed.

Results

A total of 119 pathogens were isolated from 148 donors, including 59 (49.6%) gram-negative organisms, 47 (39.5%) gram-positive organisms and 13 (10.9%) fungi. Among gram-negative organisms, the most common isolates were acinetobacter baumannii, klebsiella pneumonia and pseudomonas aeruginosa; while Staphylococcus epidermidis, Staphylococcus aureus, Staphylococcus aureus were most common detectors among gram-positive organisms. Among fungi, candida albicans, candida tropicals, aspergillus niger were most common isolates. These organisms were detected in respiratory tract, bloodstream and urinary tract. The most common pathogens in the sites above were acinetobacter baumannii, staphylococcus epidemidis and tropical candidiasis respectively.

Conclusion

The overall incidence of organisms isolates in donor organ is very high in China. The pathogens involve bacteria and fungi. It is urgent to control infection of donor organs in clinic.
