Value of pulse-indicator continuous cardiac output monitoring in management guiding of septic shock: a Meta-analysis

Abstract:

Objective

To investigate the value of pulse-indicator continuous cardiac output (PiCCO) in guiding management of septic shock.

Methods

The PubMed, CNKI, WangFang Databases were searched for the short term outcomes of PiCCO guided management in septic shock patients, compared with central venous pressure (CVP) guided outcomes from January 2011 to March 2020. Two reviewers independently screened the literatures based on the inclusion and exclusion criteria. Data were analyzed by Rev Man 5.3 software.

Results

Twenty one studies involving 1572 patients were included. Compared with CVP group, patients in PiCCO group had reduced 28 day mortality (OR=0.55, 95%CI: 0.43-0.70, P<0.001) and shortened duration of mechanical ventilation (SD=-1.77, 95%CI: -2.36 - -1.19, P<0.001).

Conclusion

In septic shock patients, managements guided by PiCCO has an advantage over conventional CVP with decreased 28 day mortality and shortened duration of mechanical ventilation.
